There is a lack of evidence on psychopharmacological treatment for patients with extreme anorexia nervosa (AN); however, these patients seek medical help in mental health settings. There is a need for information on treatment approaches for this group of patients.Objective %u2014 to analyze psychopharmacological treatment patterns of inpatients with AN with extremely low body mass index (BMI) < 13 kg/m2during hospital treatment and to assess treatment outcomes in 1%u20132 years after hospital discharge. Methods. Retrospective medical chart review (n = 47) with patient follow-up assessment at 1%u20132 years after discharge. Treatment outcomes were assessed using: body mass index (BMI) at discharge and follow-up, occupation, quality of life (QoL), satisfaction with treatment, mortality. Quality of life was assessed using World Health Organization Quality of Life Questionnaire (brief version). Treatment satisfaction was assessed using Satisfaction with Hospital Treatment Questionnaire. Results. All patients received complex therapy; olanzapine was the most frequently prescribed antipsychotic (54.5 %), mirtazapine was the most frequently used antidepressant (57.1 %), and hydroxyzine was used in 55.3 % of patients. At the discharge 63,8% patients achieved BMI > 16 kg/m2and 72.7 % had BMI > 16 kg/m2 at the follow-up. All patients were alive at the time of follow-up, 88.9 % were occupied. Age and illness duration negatively correlated with BMI at discharge (r = %u20130.29 and r = %u20130.31, respectively, p < 0.05), but not with BMI at follow-up. Quality of life exceeded the population norms. Patients were highly satisfied with the hospital treatment (satisfaction rates exceeded 70 %).Conclusion. Olanzapine, mirtazapine and hydroxyzine are the most commonly prescribed drugs for the treatment of extremely underweight patients with AN.
